Transition Consults LLC is a group of passionate professionals, working in disability services throughout Pennsylvania, since the 1980s. Our brokers are certified to provide participant-directed supports brokering in the Commonwealth. Through support and education, Transition Consults gives people the opportunity to self-direct their lives, identify their passions and gifts, choose services, community and jobs that meet their needs and interests.
