Transition Consults LLC is a group of passionate professionals, working in disability services throughout Pennsylvania, since the 1980s. Our brokers are certified to provide participant-directed supports brokering in the Commonwealth. Through support and education, Transition Consults gives people the opportunity to self-direct their lives, identify their passions and gifts, choose services, community and jobs that meet their needs and interests. We believe that all people have the right and responsibility to be an active part of their community, regardless of their ability or disability.
