Urbandale Health Care Center is a 106-bed skilled nursing and rehabilitation facility that offers a full spectrum of post-hospitalization care including TruRehab therapy services, short-term skilled care, and long-term care. We are conveniently located one block East from the corner of Meredith Drive and 86th Street. Urbandale Healthcare Center is positioned to serve the residents of Urbandale, Johnston, Grimes and surrounding communities. It’s All About U! Urbandale is a privately owned skilled nursing facility that offers a full spectrum of care including rehabilitation, short term skilled care and long term care. We provide 24-hour medical care and support services to our residents.
