Dynamic Health Systems created VitruCare as an innovative patient engagement platform to be used in healthcare settings that was created to address the challenges of providing personalized care to patients with chronic conditions or other conditions involving complex care plans. The platform was developed by a team of healthcare experts in collaboration with their patients who recognized that engaging patients in their care and motivating them to take an active role in managing their condition has a very positive effect on health outcomes resulting in significant time savings for healthcare professionals and future healthcare costs. With the Platform Dynamic Health Systems assists healthcare service providers to achieve digital transformation in two critical areas: 1) Innovating the way patients take part in their healthcare. We call this PEACH (Patient Engagement and Activation in Co-creating Health) 2) Innovating the way that care professionals collaborate with patients and each other. This we termed CIHWI (Care Integration and Health Work Innovation) This "fruit salad" approach has helped several healthcare service providers to achieve major clinical and operational benefits and win awards in health innovation.
