Colorado Health Network is a statewide nonprofit with offices in Denver, Fort Collins, Colorado Springs, and Grand Junction. CHN also has satellite offices in Greeley and Pueblo.
The mission of Colorado Health Network is to equitably meet the evolving needs of people affected by HIV and other health conditions through prevention, care, and advocacy.
Colorado Health Network, Inc supports over 5,000 clients living with HIV and other health conditions across Colorado to access our comprehensive support services including Medical and Oral Health Care, Case Management, Behavioral Health and Substance Abuse Counseling, Housing/Financial Assistance, and Food Banks. Each supported service includes the ultimate goal of working with individuals is to provide linkage to care so they can make healthy choices, increase their stability, and access healthcare. CHN also provides regular HIV/HCV and STI testing to ensure as many people as possible are aware of their status and linked to medical care.
