Action Wellness enhances the lives of individuals living with HIV and other chronic illnesses by providing a range of trauma-informed social services to help our clients achieve physical, emotional and economic well-being. We offer individualized services to over 1600 individuals and their families that include: • Medical Case Management • Linkage and Engagement Services After Release from the Philadelphia Department of Prisons (LEAP Program) • Housing Assistance • Buddy Services • Prenatal Support • Immediate Seating – a program offering free tickets to plays, musicals and other special events for clients. Action Wellness staff, volunteers and board members work together to sustain and enhance the quality of life for the diverse communities it serves.
