The Nassau Queens Performing Provider System (PPS) is one of 25 New York State regional health care networks that were selected for funding under the New York State Delivery System Reform Incentive Payment (DSRIP) Program–a program that aims to promote better care coordination and reduce avoidable hospital visits by Medicaid beneficiaries by 25 percent over five years, and also aims to address gaps in health services, care delivery and social needs.
The Nassau Queens PPS is led by NuHealth (Nassau University Medical Center), in alliance with Northwell Health’s Long Island Jewish Medical Center and Catholic Health Services of Long Island. These three principals have a long history of serving the health care needs of the communities they serve.
