Leicester City South PCN is a an agile forward thinking primary care network delivering care to patients across 5 general practices: Saffron Health, The Hedges Medical Centre, Dr G Singh Sturdee Road Health and Wellbeing Centre; Inclusion Healthcare; The Assist Practice. Our vision is to continue to improve the quality of care that we provide in alignment with the needs of our patient population. We do this by building on the existing foundations of care within general practice and enable greater provision of proactive, personalised, co-ordinated care as well as integrated health and social care for people within our community.
