Horizon Care Center is a 280 bed Skilled Nursing Facility. We provide Physical Therapy, Speech therapy, Occupational Therapy, and Wound Care. We also have a Dementia Unit, and we offer Hospice and Palliative Care. Each resident has an attending physician that will visit the facility on a regular basis.
