Established in 2007, the Hospice Foundation (HF) is a supporting foundation of Center for Hospice Care (CHC) which was established in 1978 as an independent, community-based, not-for-profit organization. CHC is dedicated to improving the quality of living through hospice, home health, grief counseling, and community education. With offices in South Bend, Plymouth, Elkhart and Mishawaka, Center for Hospice Care serves Elkhart, Fulton, Kosciusko, LaGrange, LaPorte, Marshall, St. Joseph and Starke counties in Indiana.
In its efforts to support the expansion of hospice and palliative care, HF also supports the work of various related national and international organizations, including the National Hospice and Palliative Care Organization (NHPCO), National Hospice Foundation, Global Partners in Care and the Palliative Care Association of Uganda.
