At D&S, our mission is to empower individuals to live a life of independence and self-determination. One of the ways we realize this goal is through Person Centered Thinking, an ongoing process that uses a variety of skills to discover what is important to an individual – such as activities, places, things, and people, and what is important for an individual to help them live a better life. From this specialized discovery process, a person-centered description of the individual is created. This then, leads to an action plan that builds on their strengths, talents, and skills and works toward realizing their goals while maintaining their health and safety.
D&S’ in-house team of psychology professionals work closely with individuals day-to-day to establish support services and track progress over the long term.
Each individual at D&S partners with a team of nurses who provide ongoing medical oversight. This one-on-one relationship coupled with a low nurse-to-individual ratio ensures the specific needs of each individual are met.
D&S provides a full array of day-to-day health and specialized medical services to help all individuals, regardless of disability or health status, maintain the highest level of independence and quality of life possible.
D&S support oversight professionals work side-by-side with the individual throughout all service planning and support procurement, and help ensure their happiness and success in the community.
