About REACH: A community healthcare project REACH is a volunteer community project developing a primary healthcare service for Melbourne residents. The project is driven by its volunteer membership – students and recent graduates of healthcare disciplines at the University of Melbourne. Students and recent graduates come from a variety of disciplines, including Social Work, Nursing, Medicine and Physiotherapy. Our goal is to develop a comprehensive, interprofessional healthcare service that works with the community to meet their needs. As a student-driven project, we have designed this healthcare service according to the complex health needs of our clients in the community. We also consider the interprofessional and community practice learning needs of our students. Currently our main method of service delivery is the REACH Clinic. We plan to extend community healthcare services beyond clinic hours in the future. Model of Care: “Collaborative Practice” REACH has developed its own interprofessional model of care, which we call Collaborative Practice. This model of care is a unique system of clinic operations and education that promotes teamwork among the disciplines to improve client health outcomes.
