Founded over 40 years ago as a Federally Qualified Health Center, HRHCare Community Health is one of the nation’s largest community health providers. The HRHCare network of 28 health centers throughout the Hudson Valley and Long Island provides affordable, accessible care to over 175,000 patients through more than 650,000 visits annually. HRHCare health centers provide primary and preventive treatment including, but not limited to: adult and pediatric care; obstetrical and gynecological care; dental services; behavioral health ; nutrition counseling; HIV care; health education; and a full complement of outreach, translation, transportation, benefits counseling and health insurance enrollment assistance. Excelling in patient care, our network has received the Gold Seal of approval from the Joint Commission, the highest level of Patient Centered Medical Home recognition from the National Committee for Quality Assurance, and is a designated Planetree affiliate.
