Our hospice team makes people comfortable in the last six months of their lives by honoring the patient’s wishes and by providing far more than medical care. We take into consideration patients' medical, emotional and spiritual needs so they can more fully enjoy time with loved ones. We provide these services in homes, in acute and chronic care facilities, and at our six-bed residence. Following a patient’s death, we also support the family with 13 months of bereavement services.
Our palliative care team focuses on two dimensions of caring for people with life-limiting illnesses. First, we focus on the patient, the caregivers and the family rather than on the disease. Second, we focus on relieving the burden of illness, enhancing the quality of life, and fulfilling the patient’s goals for comfort.
Finally, as a part of our commitment to the community, we provide bereavement support services to anyone in our service area who is grieving the death of a loved one, whether or not they died on hospice.
