Family Health Teams are a new way of providing health care to patients and their families in Ontario. It is a team approach that brings together family doctors with other health care providers in order to coordinate the best possible care for patients. Everyone on the team works together to meet the patient's needs. Our focus is on keeping you and your family healthy, not just treating you when you are sick.
A Family Health Team combines the expertise and services of different health care providers to meet many kinds of needs. In addition to family doctors, the team includes registered nurses, nurse practitioners, pharmacists, dietitians, mental health workers, social workers and other health care providers. Each Family Health Team is unique. It will vary in size and composition, depending on available resources, the needs of the community, and the number of providers available to participate.
