We are a boutique consultancy based across the north of England who provide specialist advice and services to individuals and families, regarding making an application for NHS Continuing Healthcare. We help individuals and /or families who; - Unwell and need care, but are being placed into Social Care and are being asked to pay for this care (which is means tested). - Already in the Care system receiving social care and paying fees (such as Care Home) and would like to understand how NHS Continuing Healthcare works so that the NHS may take over paying the fees. - Already receiving a NHS Nursing Care contribution, but it is felt that their care needs are great than being that provided by the registered nurse. - Have already applied for NHS Continuing Healthcare and have been unsuccessful, but have a primary healthcare need that they feel is being overlooked. Our services include; - Full end to end overview of the NHS CHC process. - Application for NHS CHC and the Checklist Toolkit - Support with completion of the NHS CHC Decision Support Tool. - Representation and preparation for the Clinical Commissioning Groups MDT panel. - Ongoing support for 3 month and annual review.
