The Healthy Communities Foundation Australia is working to help transform health care and community services in partnership with communities across Australia. Established as a charity in 2001, the Foundation developed the 'walk-in, walk-out' model of rural and remote health care. Today it is one of the largest NGO providers of primary health care in Australia investing more than $130 million into local community health initiatives and programs including Aboriginal community development for health, youth suicide prevention and fetal alcohol syndrome.
