Southern Melbourne Primary Care Partnership (SMPCP) is one of 28 Primary Care Partnerships (PCPs) across Victoria.
Funded by the Victorian Government, PCPs are made up of a diverse range of member agencies who work together to improve local outcomes.
Cor members of the partnerships are generally hospitals, community health, and local government with mental health, alcohol and other drug, disability and women's health services also commonly participating.
The SMPCP catchment covers the local government areas of Bayside, Glen Eira, Kingston, Port Phillip and Stonnington.
