PACE (Program of All-inclusive Care for the Elderly) is a unique program of long term care that allows elderly adults to remain in their home. We work with you and your loved ones to provide the support at home, medical treatment, and social interaction needed to remain in the community you love.
The key to your success is our care team. This is a team of physicians, nurses, pharmacists, social workers, therapists, and transportation staff who are familiar with you and your needs. The care team meets daily to discuss all aspects of your care and provide services when appropriate. Our physician is a geriatrician specializing in the care of adults as they age.
We encourage caregivers to remain involved by providing frequent communication and supportive services including respite care.
PACE of Southwest Michigan serves Berrien, Cass, and Van Buren Counties.
