Community Healthcare of Texas has been providing hospice home care, inpatient care, and respite care since 1996 to patients that have been diagnosed with a life-limiting illness and given a prognosis of six months or less. All care and support are provided wherever our patients call home - a personal residence, assisted living facility, nursing home, hospital, or one of our three home-like hospice inpatient facilities. Our Hospice Interdisciplinary Team of physicians, nurses, social workers, chaplains, and nurse aides work closely to provide the best plan of care to meet the needs of our patients and families.
Community Healthcare of Texas also provides perinatal and pediatric hospice and palliative care programs for those with a serious illness that are not ready hospice.
