Sherbourne Health is a dynamic provider of integrated health services, community programs and capacity-building initiatives that enable people and diverse communities to achieve wellness.
Sherbourne has been serving the diverse communities of southeast Toronto and beyond since 2002. In addition to providing primary care, we offer our clients mental health services in both individual and group settings, outreach support, complementary care (chiropractic, acupuncture, naturopathy), and chronic illness management services.
We are widely recognized for our efforts to reduce social, economic and other barriers to health care. For example, our mobile bus program delivering health care directly to people outside our walls and our Acute Respite Care (ARC program for homeless and under-housed people are unique Canadian programs that have brought us international acclaim. As well, our leading-edge health and support services for lesbian, gay, bisexual, trans, 2-Spirit and queer (LGBT2SQ) communities attract attention around the world.
At Sherbourne Health, we believe that to truly build healthy individuals and communities, services must extend beyond basic health care. Through key partnerships, we address the social determinants of health that are the root causes of many health problems.
Since 2007, we have been serving clients through our Family Health Team that brings together healthcare providers and practitioners from different disciplines to coordinate the best possible care for a particular patient. At Sherbourne, a Family Health Team can comprise physicians, nurses, health promoters, mental health counsellors, outreach workers, dietitians and client resource workers, all tailored to the needs of the client.
Sherbourne combines high quality clinical care with responsive, culturally appropriate community development programs that bring low-barrier services to people in the surrounding communities who need it most.
