North Star Case Management offers a bespoke therapeutic service to children and adults with complex injuries and health conditions.
Our service is provided by qualified and experienced healthcare professionals known as Case Managers, i.e. Occupational Therapists, Nurses, and Physiotherapists.

We are a professional and welcoming group of case managers with a proactive and responsive approach to our work.
The case managers actively support clients, and their families, to manage the consequences of complex long term injuries and conditions. They have extensive experience of working with individuals with acquired brain injury, cerebral palsy, spinal cord injury, orthopaedic trauma and limb loss.
We are committed to promoting clients’ recovery and wellbeing, maximising their potential, facilitating functional independence, and enabling a quality of life that brings fulfilment and life satisfaction.
Initially a case management assessment is undertaken and a rehabilitation plan is devised. Case managers then coordinate services to meet the clients’ needs in areas such as care and support, therapies and healthcare, home adaptations, specialist equipment, education, vocation, social participation and leisure.
