North Jersey Health Collaborative

The North Jersey Health Collaborative is a not for profit 501(c)(3) corporation whose partners in public health, health care, and community organizations have a shared belief that in leveraging our resources and expertise there is greater opportunity to maximize our impact on improving the health of our communities. The core functions of the NJHC include coordinating community needs assessments, health improvement planning, sharing data and resources, grant preparation, and developing and disseminating evidence based strategies to improve population health.
