Established in 1932, the Lutheran Home at Concord Reserve is a nonprofit, faith-based, Senior Life Community. Lutheran Home is a skilled nursing facility. Our beautiful 24-acre Concord Reserve campus also includes Assisted Living, secure Memory Support unit, and Center for Advanced Rehabilitation. In-house therapies include: Physical Therapy, Occupational Therapy, Speech & Swallowing Therapy, Art Therapy & Music Therapy.
