Founded in 1994 out of a local GP cooperative, London Central and West Unscheduled Care Collaborative (LCW UCC) has evolved into one of London's largest and most successful providers of Integrated Urgent Care services (incorporating 111 and GP OOH) covering a population of about 3.2 million. In 2014 LCW became a Certified Social Enterprise/Community Benefit Society providing further assurance that LCW funds are spent solely on delivering and improving services to patients.
The organisation has in its 25 year history expanded its portfolio of services and our skill mix of clinicians who now include GPs, Advanced Nurse Practitioners, Nurses, Paramedics, and Pharmacists. In addition, we have grown our non-clinical support team. Most of our work is in the urgent care environment working in partnership with Commissioners, Acute and Community Trusts, GP Federations and other health and social care organisations in the development of integrated services. Integrated Urgent Care services to residents of Inner North West London (Hammersmith & Fulham, Kensington & Chelsea and Westminster) and North Central London (Borough of Camden, Islington, Haringey, Barnet and Enfield).
GP Out of Hours services to residents in outer North West London (Brent, Ealing and Hounslow) Two Urgent Care Centres (UCC) located at Charing Cross and Hammersmith hospitals. GP practices at Hammersmith and Charing Cross hospitals. Inner North West London (Hammersmith & Fulham, Kensington & Chelsea and Westminster) Single Point of Referral, co-ordinating all referrals to the Community Independence Team
Care home / telemedicine service delivering an enhanced response to 140 care homes across all boroughs in North West London.
