Integrated Care Professionals (Icp)

We are Health Care Professionals improving the quality of life for residents by increasing access, communication, and coordination of healthcare. We've raised the bar for expectations in post-acute and long-term care. ICP is the only team of Nurse Practitioners that offers full-time coverage in a skilled nursing facility, and also provides on-call services. We work as a team alongside nursing facility staff to help meet goals for quality measures. For example, ICP Nurse Practitioners have helped dramatically reduce hospital readmissions by as much as 30%. And have seen results in less than 30 days. We also work with facilities to reduce antipsychotic drug use, narcotic reductions, and polypharmacy management.
