Guided HealthCARE (GHC) is a clinical based Nurse Care Coordination/Case Management company. Our mission is to support patients, families and the clinical team as patients with acute, chronic and complex medical conditions navigate the healthcare system. Through a holistic Nursing concept, we work to enhance quality of life, prevent unnecessary hospital re-admissions, strengthen communication, provide patient/family education, offer comprehensive resources in the community and improve overall patient outcomes while providing care throughout the life span.
