Fair Haven Community Health Care was started in 1971 by community members and volunteers who wanted to initiate health services for the Fair Haven neighborhood. The vision of the Community Health Center was born in this era and grew from the premise that health care should be a right of all people regardless of race, class, or gender, and that social justice should be intimately linked with public health.
With a commitment to serving the New Haven and East Haven communities, fair Haven takes a holistic approach to health linking medical services, wellness activities, social services, and behavioral health. Fair Haven has grown from the small store front to a complex, multifaceted primary care center that currently serves more than 17,000 patients in over 70,000 visits per year, providing care at our New Haven sites at 374 Grand Ave (our main location) and 50 Grand Ave, in East Haven, CT, plus 5 school based health centers, as well as at our satellites at Bella Vista Elderly Housing Complex and MAAS.
Fair Haven offers a full range of comprehensive services from prenatal care, care for infants and children all the way up through caring for the elderly, our oldest patient just celebrated 105 years! Throughout the past five decades, Fair Haven has served several generations of families and still maintains the family model of care as a central focus for providing care for the medically under-served and uninsured. Through constant innovation, learning, and development, Fair Haven continues to break new grounds in Patient-Centered Care, LGBTQ+ programs and Substance Abuse Treatment.
