Hospice is not a place. It is there when you need it, where you need it- whether it is a private residence, nursing facility or assisted living residence.
The goal of the hospice is to provide palliative care that addresses the physical, psychosocial, emotional, and spiritual dimensions of service for the terminally ill as an integrated whole. Continuum Care Hospice is guided by the principles of responsive care, teamwork, leadership, and stewardship. Care to the patient and caregivers will enable those served to experience death with grace and dignity.
In addition to helping ease pain and other symptoms, your professional hospice team:
-Works with you, your family and your physician to provide the medical, psychosocial, emotional and spiritual care you need, when and where you need it. Our nurses, pastoral (spiritual) counselors, social workers, hospice aides, trained volunteers, bereavement experts and others stand ready to help 24/7.
-Allows you and your family to make your own decisions, respecting your beliefs and goals.
-Helps you and your loved ones through the grief process.
-Provides bereavement services.
Our hospice care focuses on maintaining quality of life. That means helping you or your loved one live in comfort and dignity at home or in a home-like facility close to family and friends.
You don’t need to manage terminal illness alone. Together, we can help you or your loved one live as fully and comfortably as possible.
