For 30 years, CHNW clinics have been providing Washington state residents with affordable, community-based care. Since the beginning, we have been advocates for healthy equity and for holistic care rooted in our knowledge of local communities and informed by our understanding of social determinants of health (SDoH), defined as the conditions in environments where patients live, work, learn and otherwise spend their time. Conditions such as food insecurity and a lack of access to support networks can impact a patient’s health.
We work to understand the SDoH that impact our patients and their communities and to account for their unique cultural needs when providing health care. We remain committed to serving all of our communities, including immigrant and refugee populations, regardless of race, class or ethnicity, because we believe no one should be without health care.
Our track record of community organizing and effective care management has made us a leader in Washington state’s health care sector. As a health network, we bring together Community Health Centers (CHCs) and other health leaders with our shared values of equal access to care and social justice. As members of Washington state’s vibrant communities, we understand the challenges that our fellow Washingtonians face when attempting to access and finance care in the midst of our busy lives. Our Cascade Select plans are powered by Community Health Plan of Washington (CHPW). As CHPW’s parent organization, we are partnering with CHPW to utilize its Cascade Care Affiliate Network and full suite of plan services for our Cascade Select Gold, Silver and Bronze offerings.
