CHIP of Roanoke Valley is an early childhood home visiting program that pairs children, from birth to kindergarten-entry, with a Community Health Nurse and Family Case Manager. We provide health care coordination, including access to medical services, chronic condition management, and preventive care like fluoride dental varnish. We also offer developmental education, kindergarten preparation, and regular child assessments.
Families receive services such as anticipatory child development guidance, employment and education support, parent meetings, and transportation, all designed to build parenting capacity and support families as they set and achieve their goals. Additionally, CHIP offers specialized case management services for prenatal care, mental health, and asthma management to address specific needs.
