Recognizing the specialized and growing needs of people living with HIV, the Area Agency on Aging created a separate case management program to serve this unique population. Care Directions® brings a focused sensitivity and expertise to assisting men, women, and children challenged by the HIV virus.
Care Directions® provides one-to-one assistance in addressing a client's particular needs. Each client is assigned a case manager who will work with the client to assess his or her situation and identify the resources available. Together, they will design a "care plan" tailored to the client's specific circumstances. Care plans may include referrals to physicians, adult day health care, in-home nursing, meals, legal counseling or transportation services as well as assistance in accessing entitlement programs, paid medical care programs, affordable housing, part-time work, or vocational rehabilitation. Client care plans are as unique as the client, and are designed to be flexible to the client's changing needs.
Care Directions® services are available in Maricopa County to men, women, and children of all ages living with HIV. Through a partnership with Mosaic, a Pinal County Long Term Care program, services are also made available to Pinal County residents.
