Active Day offers a day program designed to meet the unique needs of those who require health care and support services as a result of chronic conditions, such as: COPD; stroke; Alzheimer’s and other related dementias; seizures; diabetes; Parkinson’s; Multiple Sclerosis; hypertension and heart disease; developmental and intellectual disabilities. Through our specially trained staff, we are able to effectively manage these chronic conditions, allowing the member to prolong their independence and remain at their home in the community.
